Quantum provides a broad range of family-focused services to support young people across Gippsland:
The Home Based Care (HBC) program provides short and long term placements with approved caregivers for children and young people who are in need of out of home care. These placements are usually arranged through DHS and are often as a result of family breakdown, violence or other issues making it difficult for the child to live at home.
The objectives of the HBC program are:
- To ensure that young people are placed in home based placements that meet their emotional, physical, educational and social needs.
- To provide a high standard of care and protection to all young people.
- To support young people to participate in education, employment and the broader community.
- To assist young people to develop skills in preparation for independence and access to community information and resources.
- To support young people to fully participate in decisions that concern them and to ensure that the young person’s privacy and dignity are maintained.
- To assist the young person to maintain and enhance relationships with his or her family.
The Adolescent and Family Support Program is a group of programs providing family focused support to young people from across Gippsland. The Program’s primary aims are to improve family relationships and communication, assist young people and their families to remain living together where possible, prevent or minimise the need for Child Protection or correctional involvement, and in particular the need for substitute care or a statutory order.
These include:
- Reconnect which helps young people who are homeless or at risk of homelessness to become more involved with their family, work, education, training and the community. This involves intensive work with both the young person and their family using different approaches including mediation, counselling and practical support for the whole family.
- Finding Solutions is an intensive support service intended to divert or minimise Child Protection’s intervention into the lives of young people. Employing an assertive outreach model, this program works with young people and their families to build better relationships through mediation, family meetings and the provision of one-on-one support to the young person.
- Adolescent and Family Support is a more generalist outreach service that provides support to young people aged between 12 and 18 years who are living at home, independently, or in a placement.
- Leaving Care is a regional service aimed at achieving better outcomes for young people aged between 16 and 21 years, who are leaving or who have exited out-of-home care. This is achieved by…
- assisting them in setting up and maintaining appropriate accommodation,
- development of independent life skills,
- assist young people in identification of employment /training opportunities and their preparation and readiness for this,
- provide linkages to appropriate supports for successful transition for these young people including linking to appropriate mentors to support them through their journey.
The Leaving Care Program assists young people to develop healthy community links, including employment, training, recreational and other services identified on an individual basis to enhance their opportunities for a positive experience transitioning to independent living.
- Youth Justice Community Support Service is an integrated regional response to the provision of services to youth justice clients. Quantum has the lead role within a consortia developed to provide broad geographic service across Gippsland. The YJCSS works closely with staff from the DHS Youth Justice Unit to:
- Minimize reoccurrence of their offending behaviour,
- Support and assist these young people to access allied youth appropriate services, and
- Provide holistic support to enhance their well
